Family Support Services Coordinator

Overview:
At ACCORD we work together to help people live their greatest lives. Accord is currently looking for a Family Support Services Coordinator. As a Family Support Services Coordinator, you will partner with management in outreach efforts to families in need of services. In this role, you will help process new clients by taking their information and organizing it in a way where it can be clearly reviewed by our team. You will also be working closely with the Program Managers to ensure our programs and services for the participants are organized and on schedule so that we can provide excellent services to those we serve.

Responsibilities:
Essential Responsibilities: Duties included but not limited to: Assist in recruitment efforts Be the point of contact for new participants in the program where you will explain the Personal Support services provided by ACCORD. Organize initial meetings with new families to the program with the assigned Program Manager. Manage multiple documents for various participant accounts- ensuring each has correct information and data entry into multiple state systems. Communicate with internal and external partners, participants and families of participants with updates, changes and information about their services on a regular basis. Problem solve issues and provide solutions to participants and team members in order to improve processes.

Requirements:
All new hires will be required to provide proof of Covid 19 full vaccination or have an approved request for exemption before their start date. Bachelor's Degree in Human Services, Social Work, Psychology, Sociology, or a related field. If you hold a Social Work degree, we expect you to be currently licensed as a Social Worker as required by the Minnesota Board of Social Work. OR a High School Diploma/ GED with 4+ years of work experience in the Mental Health/Disabilities social work.
