Case Manager - CADI/BI Waiver

Overview:
Fraser seeks qualified and dedicated Case Managers to work with individuals receiving CADI & BI Waiver services. The Case Manager serves as an advocate for clients and plays an integral part in connecting clients to resources and services in the community.

Responsibilities:
The Case Manager provides client-centered case management services to individuals receiving CADI/BI waiver services. Case Managers typically have about 40-45 clients on their caseloads and meet with clients three times per year or once every four months. The Case Manager is responsible for care coordination, monitoring health and safety needs of clients, monitor services to ensure they are effectively meeting the client’s needs and intervening as necessary. Caseloads are typically a majority of adults, with a small mix of children as well.  Fraser offers: Salary range between $45,000 - $48,000 annually  $1,000 Hiring Bonus! This time is typically split between in office and remote work, at this time Case Managers have the option to work remotely up to 4 days per week. Travel to meet with clients is required, client meetings typically occur 2-3 times per week. Case Managers meet with clients in their assigned county (either Dakota, Hennepin, or Ramsey).

Requirements:
Education requirements: You must have one of the following: Graduate from an accredited four year college/university with a major in any field. If degree is in any field, one year of full-time experience working in a social work capacity required. Graduate from an accredited four-year college with a major in social work, psychology, sociology, or a closely related field. If degree is in social work, social work licensure is required. A minimum of one year of experience working with individuals with developmental disabilities and/or mental health diagnoses strongly preferred. One to two years of experience working in service coordination or case management preferred. Other Requirements: Valid Minnesota Driver’s License with acceptable record. Effective communication in the English language, including verbal and written communication with both internal and external contacts. Ability to perform basic mathematical skills, including accurate billing. Ability to understand and implement all related regulations, policies and procedures. Ability to recognize problems, establish facts, and draw valid conclusions. Computer proficiency using Word and Excel with proficiency in keyboarding skills. Excellent organizational skills and attention to detail. Basic mathematical skills, including the ability to add, subtract, multiply, divide and calculate percentages.
