Overview:

Responsibilities:
The RN Case Manager or RN Visit Nurse will perform nursing care services to adults with post-acute and chronic health conditions, striving to attain high client satisfaction and quality care outcomes. Person-centered nursing services are provided under the health care provider’s orders that are in accordance with federal, state, and local laws and agency policies. Nurses work under the supervision of an RN Clinical Manager. Prior experience working with adults is preferred. Patient types are adults with medical, mental health, orthopedic, cardiac, and neuro health conditions.
Requirements:
Associate or Bachelor’s Degree in nursing from an accredited school of nursing. Current or pending State of Minnesota RN license. Proof of Covid-19 full vaccination or exemption documentation before start date. One (1) year professional nursing experience in home health preferred. RN nursing skills in assessment, care planning and evaluation, treatments, and documentation.
