Overview:
Living Well Disability Services delivers exceptional services that transform the lives of people impacted by intellectual, developmental and physical disabilities. Services are available to people in their own home, their family’s home or in one of our 36 group homes throughout the 10-county region. We offer a broad spectrum of people with intellectual, developmental, and physical disabilities such as cerebral palsy, Down syndrome, multiple sclerosis, autism, and brain injury. These may be complicated by a variety of chronic health conditions. Innovation in service, accessibility, wellness, and technology help people pursue their goals and live as independently as possible.

Responsibilities:
Lead and support to a diverse team of direct support professionals, CNAs, Assistant Program Managers and Advanced Assistant Program Managers by providing effective hiring, training, supervision, mentorship and job performance evaluation to ensure successful implementation of services. Assess for design and assure implementation of individualized services that are responsive to directives from people receiving services, their legal representatives and interdisciplinary teams. Provide, monitor and evaluate services to assure quality standards are maintained and effective through the use of quality improvement tools. Manage service location to assure that contractual, financial and human resource outcomes are met by leading effective communication with staff, co-workers and people receiving services and their interdisciplinary team members, managing the purchase process to ensure adequate supplies are available and budget targets are met, and setting staff work schedules that meet the needs of people receiving services. Complete all tasks and responsibilities necessary to comply with all licensing requirements, including analyzing, preparing and submitting reports and communicating other information to the appropriate person, department or agency. Complete required trainings as scheduled, attend staff meetings, and maintain certifications for essential job functions (CPR, First Aid, Med Administrations, and/or Mandt). Complete other tasks and responsibilities as assigned and/or needed to ensure the agency meets its mission.

Requirements:
Have the passion, willingness and courage to make a difference in the lives of persons who have disabilities. Four-year post-secondary degree and QIDP credential; or Bachelor’s Degree in a field related to human services and one year of full-time work experience providing direct care services to persons with disabilities or persons 65 and older; or an Associate Degree in a field related to human services and four years full-time work experience providing direct care services. Valid Driver’s License Pass a background check through MN Department of Human Services
