Group Home Coordinator Coverage Specialist

Overview:
Fraser, a Minnesota leader in healthcare, housing, education, and employment services for individuals with special needs, is seeking a passionate, flexible, and dedicated individual to provide service coordination and float coverage for the Supervised Living program. In this position, you will oversee coordination and supervisory responsibilities at one consistent group home location while flexing the other half of your schedule to provide support for open positions, employee leaves of absence, or coverage for Coordinators who are out of the office. This role provides supervision and leadership to assure continuous quality improvement in active treatment service development and implantation. Applicants with strong communication skills and a variety of direct care experience are strongly desired.

Responsibilities:
Responsibilities:Day to Day Site Management-Assure availability as required, either at the work site or electronically, to respond to emergencies immediately and provide direction and support to staff and clients.-Serve as the primary float staff to multiple locations across the Twin Cities south metro. Provide coverage for both vacant positions and Program Coordinators who are out of the office or on various forms of employment leave (including medical, workers compensation, personal, funeral etc.)-Assure necessary staff coverage, and recommend staffing, to meet the needs of the clients served.-Assess customer satisfaction, service delivery excellence, and regulatory compliance, and respond to opportunities for improvement, through implementation of a quality assurance process.-Assure correctly prepared and accurate medication sheets, and the accuracy and documentation of administration of medication and treatments. Coordinate medication ordering.-Assure required documentation for all areas of client care are completed as required, including but not limited to Health Progress Notes, appointment forms, narrative notes, and data collection.-Coordinate weekly meal planning, grocery shopping and meal preparation as directed for each client served.-Maintain accurate financial information for sites supervised and all clients served. This is a salaried, exempt position.

Requirements:
Education and Experience:Experience requirements vary based on level of education. Program Coordinator qualified as defined by rule 245D:-BA/BS in Human Services or related field with 1 year of full-time work experience (or equivalent) providing direct care services to persons with disabilities or persons age 65 years and older OR-Associate degree in Human Services or related field with 2 years of full-time work experience (or equivalent) providing direct care services to persons with disabilities or persons age 65 years and older OR-A minimum of 50 hours of education and training related to human services and disabilities with four years of full-time work experience providing direct care services to persons with disabilities or persons age 65 or older until the supervision of an individual qualifying as Coordinator under 245D.Other Requirements:-One year of leadership or management experience in a group home setting required, with two or more years highly preferred-Working knowledge of regulations 245D and others pertaining to residential services required-Ability to recognize problems, establish facts, and draw valid conclusions to solve problems required-Valid driver’s license with acceptable record and access to a reliable vehicle required-Basic mathematical skills with ability to accurately manage a budget and create financial reports-Proficiency in using Microsoft Office programs including Excel, Word, and Outlook
