Physical Therapist- $1250 New Hire Bonus
Overview:
The Licensed Physical Therapist (PT) will provide safe and effective intermittent home-based physical therapy visits to home health clients per the physician/health care provider’s orders and as defined by the Minnesota Board of Physical Therapy Practice Act. The evaluation and/or treatment of any person by the employment of physical measures, use of therapeutic exercises. rehabilitative procedures, with or without assistive devices will be for the purpose of preventing, correcting, or alleviating a physical or mental disability.
Responsibilities:
Job duties also include but are not limited to: conducting OASIS assessments, performing visit documentation, obtaining physician orders, communicating with health care professionals, instructions to physical therapy assistants and home health aides. The physical therapist reports to the Home Health Clinical Manager.
Requirements:
Bachelors Degree or higher in Physical Therapy from an accredited educational institution.Active Physical Therapist license in the State of Minnesota with a minimum of 6-12 months clinical physical therapy experience.A minimum of 6 months experience providing physical therapy servicesA minimum of 6 months of physical therapy experience in a Medicare certified home health agency.
