Job Title:
Licensed Practical Nurse (LPN)
Overview:
As a Licensed Practical Nurse (LPN), you will perform skilled home health nursing duties in accordance with the federal, state and local laws and policies of Accord Home Health. The agency provides services to adults with various cultural backgrounds with mental health, chronic, and post-acute care conditions. Uses the nursing process to provide intermittent skilled home care visits in the Twin Cities metro area, in accordance with the Minnesota Nurse Practice Act and under the supervision of the RN Clinical Manager. Experience working with adults with medical and mental health conditions required.
Responsibilities:
LPN Services may include but are not limited to: medication dispensing and management, injections, lab draws/collection, vital signs, health information sharing, observation and monitoring of health status, wound care, and special treatments. Duties also include: nursing visit documentation, physician orders, and communication with health care professionals.
Requirements:
Graduate of an accredited school of professional practical nursing.Current license to practice as a Licensed Practical Nurse (LPN)Minimum of one (1) year of experience in care setting or equivalent experience.Skilled in nursing observation, treatments, medication management and administration, falls prevention.Effective written and verbal communication skills and good interpersonal skills.
