Assistant Program Manager

Overview:
What is an Assistant Program Manager? An Assistant Program Manager is someone who works to empower and support the people we serve to achieve their goals and dreams. An Assistant Program Manager provides leadership to support professionals and is actively engaged as an effective and cooperative member of service and interdisciplinary teams to implement people’s services plans and encourage people to make choices, decisions and plans that reflect people’s personal interests.

Responsibilities:
Major Position Responsibilities Provide direct and personal care, and support with activities of daily living (ADL) to people receiving services and provide leadership and direction to other staff providing services. Activities may include, but are not limited to: Establish and prioritize work schedules and shift assignments related to direct care, support people with daily routines and activities, manage and coordinate people’s activity and transportation schedules, manage medical needs and coordinate care in conjunction with nursing staff, provide positive examples and appropriate support regarding interpersonal behavior and daily living skills. Maintain an environment that is safe, clean, comfortable and supportive of people receiving services. Activities may include, but are not limited to: fix or report items that need repair, ensure sidewalks/driveway/yard/gardens are maintained year-round, establish cleaning schedule, manage household and medical equipment supplies, oversee COMPASS, plan meals and purchase groceries, and manage vehicle maintenance schedule. Assist Program Manager with managing staff by supervising shifts, providing input on performance reviews and coaching /disciplinary actions, and providing leadership, training and support to staff. Engage in respectful, effective and proactive communication with people receiving services, team members, coworkers, families, work/day programs, case managers and others. Assist Program Manager with house and people’s financial needs by managing month-end financial processes, managing time sheets and attendance, and assisting individuals with personal finances. Manage Person Centered Planning/ Coordinated Service and Support Plan or CSSPA process and paperwork for people receiving services who do not have Advanced Direct Support Professionals, and support and train other Advanced Direct Support Professionals. Participate in organizational management and training efforts by serving as a trainer (CPR, Mandt, Advocacy, etc.) and/or participating in internal and/or external committees and task forces as requested. Perform responsibilities in a professional manner, establish and maintain professional boundaries in all interactions and communications between team members, families, co-workers, individuals, and support staff. Complete required trainings as scheduled, attend staff meetings, and maintain certifications for essential job functions (CPR, First Aid, Med Administrations, and/or Mandt). Complete other tasks and responsibilities as assigned and/or needed to ensure the agency meets its mission.

Requirements:
Skills, Knowledge and Abilities: The requirements listed below are representative of the knowledge, skill, and/or ability required to perform the Advocate job effectively. Two years relevant post-secondary education or equivalent experience preferred. Have the passion, willingness and courage to make a difference in the lives of persons who have disabilities. Have the commitment to be part of an effective team that is accountable, supportive and responsible to one another as well as the willingness and confidence to learn to work independently. As a learning and evolving organization, each team member should have the willingness to embrace change in order to meet the needs of those we serve. Demonstrated proficiency in a leadership role working with people with developmental disabilities or related conditions; or equivalent experience that relates to the specific needs of people receiving services. Demonstrated proficiency in Coordinated Services and Support Plan/ CSSPA development, implementation and documentation. Demonstrated ability to work independently. Ability to communicate effectively verbally and in writing using the English language. Ability to implement and document services provided. Ability to lift and/or move up to 32 pounds regularly and 100 pounds occasionally. Valid driver's license and approved to drive for Living Well Disability Services required. Ability to obtain Medication Administration Certification, CPR Certification, and MANDT Certification within 2 months after hire and maintain those certifications while employed at Living Well Disability Services.
