Overview:
The purpose of this position is to provide personal cares and delegated nursing services to maintain the client's physical and emotional well-being. Traveling to client's homes in the Coon Rapids area.
Responsibilities:
Assist the client with personal cares as identified in the assignment sheet/client care plan. Cares completed may include all or some of the following: assisting with dressing, grooming and oral cares, shampooing of hair, assisting with showering/bathing, applying lotions, fingernail care and foot care and other cares as assigned. Assisting clients with use of the commode/urinal/toilet and with adult incontinence products as assigned. Perform delegated nursing services as assigned by RN Manger Medication reminders, blood glucose monitoring, blood pressure and catheter care.Complete duties accurately and timely Help each client to reach and maintain his/her highest level of performance and independence. Establish a working relationship with clients, families and related professionals while performing assigned tasks. Observe client and report to the RN, in a timely manner, changes in the clients’ physical and emotional condition such as change in attitude, appearance, loss of appetite, weight, etc. Document services provided accurately and consistent with agency policies and complete all required paperwork in a timely and legible manner. Attend required in-service training – at least 12 hours for every 12 months of employment as well as staff meetings and other agency events. Performs light housekeeping duties, including vacuuming, dusting, sweeping, washing dishes, making beds, washing floors and doing laundry. Follows established policies and infection control procedures. Assumes responsibility for establishing and maintaining effective working relationships with all staff members and management. Follows safe patient handling procedures and established safety practices for using equipment.
Requirements:
Must be 18 years of age or older.Possess a valid driver's license with a good driving record and have reliable transportation.The ability to pass a background check.A willingness to work hard and a desire to help others.Strong verbal and written communication skills.Basic computer skills.CNA Certification required .
