Join a good team! Various shifts available. What is a Certified Nursing Assistant (CNA)? A CNA is someone who has completed the coursework and is certified to work as a CNA. A CNA works to empower and support the people we serve to achieve their goals and dreams. A CNA is actively engaged as an effective and cooperative member of service and interdisciplinary teams to implement people’s services plans and encourage people to make choices, decisions and plans that reflect people’s personal interests. A CNA is committed to providing services in a manner that reflects Living Well Disability Services’ mission and vision.

Responsibilities:
Major Position Responsibilities Provides direct and personal care, and support with activities of daily living to people receiving services. Activities may include, but are not limited to: Prioritizes and accomplish work responsibilities related to direct care in a timely manner, supports people with morning and/or bedtime routine, successfully administers and documents medication, prepares meals and makes/documents menu substitutions, contacting nurse with immediate medical needs, provides positive examples and appropriate support regarding interpersonal behavior and daily living skills. Implements services as identified and requested by people receiving services within the community by planning, participating in, and leading community outings as required by individual needs. Conducts skills-based training on medical, personal and physical cares and mechanical equipment as requested by the Program Manager or RN/LPN. Provide training support to other employees as requested by the Program Manager or RN/LPN . Attends interdisciplinary team (IDT) meetings as requested by the facility Program Manager or RN/LPN. Completes medication checks as requested by the facility Program Manager or RN. Set up DoseHealth devices at the direction of the RN/LPN as applicable. Maintains an environment that is safe, clean, comfortable and supportive of people receiving services. Activities may include, but are not limited to: fix or report items that need repair, snow/ice removal as needed, maintain yard/gardens as assigned, complete cleaning list/responsibilities and communicate supply and equipment needs in order to successfully complete cleaning duties. Works closely with the Assistant Program Manager to ensure infection control methods are being followed in the home. Ensures all direct care staff are checking symptoms and reporting them according to guidelines. Informs Program Manager of issues and concerns. Communicates respectfully and effectively with people receiving services, team members, and coworkers by updating/reading communication book, having verbal discussions during shift changes, reading and responding to e-mails, voicemails and other requests for information. Provides quality and accurate documentation as needed, including incident reports, house logs (as applicable) and program books (journal notes, data sheets, day program books, medication administration) and manage people’s required financial reporting, such as tracking petty cash, as assigned. Successfully supports people on medical appointments in a professional manner as scheduled and follows up with nurse, documentation, ordering /transcribing medications Performs responsibilities in a professional manner, establish and maintain professional boundaries in all interactions and communications between team members, families, co-workers, individuals, and support staff. Completes required trainings as scheduled, attend staff meetings, and maintain certifications for essential job functions (CPR, First Aid, Med Administrations). Completes other tasks and responsibilities as assigned and/or needed to ensure the agency meets its mission. Requirements:
Skills, Knowledge and Abilities The requirements listed below are representative of the knowledge, skill, and/or ability required to perform the CNA Professional job effectively. Maintains certification as a Nursing Assistant and favorably registered with the Minnesota Nursing Assistant Registry. Have the passion, willingness and courage to make a difference in the lives of persons who have disabilities. Have the commitment to be part of an effective team that is accountable, supportive and responsible to one another as well as the willingness and confidence to learn to work independently. As a learning and evolving organization, each team member should have the willingness to embrace change in order to meet the needs of those we serve. Ability to communicate effectively verbally and in writing using the English language. Ability to implement and document services provided. Two years relevant post-secondary education or equivalent experience preferred (but not required.) Ability to lift and/or move up to 32 pounds regularly and 100 pounds occasionally. Maintains a valid driver's license and approval to drive for Living Well Disability required. Ability to obtain Medication Administration Certification, CPR Certification, and MANDT Certification within 2 months after hire and maintain those certifications while employed at Living Well Disability Services.
